Open Text Fields

Open text fields should include as much information as is necessary to provide context for the interaction and uniquely identify the question or activity. These fields are full-text searchable and viewable by all users, so any personal patron information must be removed. Whenever possible, include information that may help to identify resources, services, and events. 

There are three open text fields present in the form. They are:

  • Question Field: This field is expected to include what the question/activity was about. It can be a summarized question topic or a detailed question.
  • Answer Field: This field is expected to include the answer that was given to the question asked. It can be a summarized answer or a detailed answer

Time Stamp

The Time Stamp option is the radio button field that depicts the time when the transaction or the consultation took place. The Current time option takes the system's present time as the time of occurrence. The Edit Date/Time option facilitates backdating the occurrence. 

Answered By

Answered By option is a drop-down field that represents the person who entered the transaction or the consultation. It defaults to the logged-in user. 

List fields

List fields are a pre-populated list of options that help describe the transaction. 

The form has ten list fields in total. Three of the list fields are required for every transaction, they are:

  • Question / Activity
  • Method
  • Patron

There are also seven conditional list fields that are not required but may be selected to provide additional context for the interaction. The conditional list fields are:

  • Department
  • Your Role
  • Time Spent
  • Service
  • Resource
  • Technology
  • Space

Submission Options

The submit option enters the transaction into the dataset. There are two options

  • "Submit" which saves the transaction, then retains the selected list field values for the next entry.
  • "Submit & Clear" which save the transaction, then clears all fields on the form.
